Discharge
Before you go Home
Your attending physician will inform you of your discharge date.
You will be discharged directly from your ward and will receive a certificate of hospital stay as well as a discharge summary. If you require ongoing care, you will also receive a nursing discharge report. Please return any items provided to you during your stay to the ward staff before leaving.
Medications administered during your stay may only be provided for one additional day upon discharge. In exceptional cases, your physician may issue a prescription.
Deposited money and valuables can be collected Monday to Friday, from 7:00 a.m. to 3:00 p.m. at the InfoCenter in the Entrance Center â only upon presentation of the deposit receipt. Please note: We cannot accept liability for any personal belongings left behind in your hospital room.
Discharge Management
To ensure that patientsâespecially those with increased care needsâare well prepared for the time after their hospital stay, at-home care must be carefully planned in advance. In addition to the medical and nursing teams, dietitians, hospital social workers, and staff from external services (such as home nursing providers or companies offering medical device instruction) are also involved in the discharge process. The ward manager is responsible for coordinating the entire process and serves as the main point of contact for all other professional groups involved.
Discharge Against Medical Advice
You may choose to leave the hospital at your own request. However, if you decide to do so against the advice of medical staff, you will be required to sign a discharge against medical advice form. By signing this document, you accept full responsibility for any health consequences that may result from your decision.
Transport Home
If medically necessary, transport home will be arranged by your hospital ward. In all other cases, you are kindly asked to organize your return home independently and at your own expense. On the day of your discharge, you may be picked up by car within the hospital grounds. If the vehicle remains on-site for less than 30 minutes, no parking fees will be charged.
Follow-Up and Check-Up Examinations
Monitoring your recovery after discharge is very important.
After your hospital stay, you will generally be cared for by your general practitioner. If you provided the name of your general practitioner during admission, they will also receive a copy of your discharge summary. Please keep in mind that you may still require rest and recovery after leaving the hospital. Should any symptoms reappear, please consult your general practitioner.
In some cases, you may be required to return to the hospital for specific outpatient examinations. If so, you will receive an information sheet upon discharge indicating the type of examination, as well as the location and time of your appointment.
Please make sure to attend all scheduled follow-up or check-up appointments! If you are unable to attend for any reason, please inform your ward in advance.
